For your convenience, we've provided the required permission form so you can read it and fill it out ahead of time! Complete the form beforehand so that you can focus on playing when you come to visit! A parent or guardian must be the person to sign for their children! One permission form can be used for an entire family as long as the individual names of each child are listed on the spaces provided. 
This permission form MUST be filled out BEFORE your child can play at The Playhouse.

THE PLAYHOUSE MUSEUM POLICIES

HEALTH AND SICKNESS POLICYFor the safety and health of all our guests, please review:
All children must be symptom-free for 24 hours before coming to play at The Playhouse.
Symptoms include:
Vomiting
Diarrhea
Rash
Persistent Cough
A fever over 100
Obvious contagious conditions such as head lice or pink eye

This 24-hour policy is designed for the sick child’s protection, as well as for others in The Playhouse.
We reserve the right to ask any guest to leave if their child is obviously ill or exhibits any of the above symptoms while at The Playhouse.

OPERATIONS POLICIESFor your information:
For the safety of all our guests, there is NO CLIMBING allowed on any of the playhouse structures.
NO dress ups or toys are allowed in the restrooms.
Due to allergy concerns, there is NO food allowed in The Playhouse.There is a drinking fountain provided and cups with lids are ok.
